Pickup Request Form for E-Waste, Universal Waste, and Hazardous Waste

PLEASE NOTE: When requesting a pickup for a box or bucket distributed through the USC Sustainability Green Office Program please make sure that the box or bucket is full. The hazardous waste team cannot retrieve half-full, or half-empty,  containers due to the volume of waste generated on both campuses.
